What is an RMMR?
A Residential Medication Management Review (RMMR) is a medication review for permanent residents of aged care facilities. Like an HMR, it begins with a GP referral and results in a pharmacist report back to the GP — but the patient context, facility governance, and register obligations are fundamentally aged-care specific.
RMMRs are a core service for pharmacists working with residential aged care providers. They support safer use of medicines in a population with high rates of polypharmacy, psychotropic use, and transitions between hospital and facility.
How RMMR differs from HMR
HMRs target community-dwelling patients in their homes. RMMRs target permanent aged care residents and integrate with facility medication charts, nurse handover, and governance committees such as the Medicines Advisory Committee (MAC).
RMMR pharmacists often manage dozens of residents across several facilities. Software needs bulk resident lists, register tracking, due-date logic, and reporting that facility managers and GPs can act on — not just a single-patient case file.
The RMMR process and PPA claiming
The workflow typically runs: GP referral, pharmacist review in-facility, report to GP, interventions implemented, and claim lodged via PPA. Facility registers — especially psychotropic and polypharmacy — must stay current between formal reviews.
