Modules
HMR
The HMR module runs a Home Medicines Review from referral to report. You start by managing your patients and moving each one along the pipeline. Inside a case you reconcile their medicines, check for interactions, and work through a structured checklist.
When the review is done, Markoste produces a friendly patient handout and a travel log for your visit records, then lets you export everything you need.
The flow at a glance
How HMR works
Each review is a case on the pipeline. Open it to do the clinical work across a set of tabs, finish with the handout and travel log, then export and report.
Getting set up
Patient management
Add and organise your HMR patients
Add the patients you are reviewing and keep their referral details together. From here you can start a new review or pick up where you left off.
Your overview board
The pipeline
Track every review's stage
The pipeline shows each review as a card in a stage column, so you can see what is referred, in progress and complete at a glance.
Doing the work
The case drawer
Open a review and use its tabs
Click a card to open the case drawer. It has tabs for the patient summary, documents, notes, medication reconciliation, drug interactions, the review checklist and an activity log.
Building the medicine list
Medication reconciliation
Confirm what the patient is really taking
Use medication reconciliation to build an accurate, up-to-date list of the patient's medicines, resolving differences between the referral and what you found at the visit.
Safety check
Drug interactions
Check the reconciled list for interactions
The drug interaction tab checks the medicines against the MIMS data so you can review any interactions and note your recommendations.
Being thorough
Review checklist
Work through the structured checklist
The review checklist guides you through the parts of a complete HMR, so nothing important is overlooked before you finalise the review.
For the patient
Patient handout
Create an easy-to-read medicines handout
Markoste builds a friendly handout for the patient, listing their medicines with what each one is for and, where available, an image of the pill. You can export it as Word or PDF.
Tip
The handout pulls medicine details and pill images from MIMS, so it is accurate and easy for the patient to follow at home.Your records
Travel log
Record your visit travel
The travel log keeps a record of the travel for your home visits, which is handy for your own records and any claims or audits.
Wrapping up
Exporting
Export the review and handout
When the review is complete, export the report and the patient handout in the format you need โ Word, RTF or PDF โ to send to the prescriber and the patient.
